Notice of Data Incident

We are providing notice of a data security incident involving Wolfe Clinic, P.C. (“Wolfe Eye Clinic”) that may have resulted in the unauthorized access to the personal information of certain individuals, including past and present patients. We take the privacy and protection of patient and employee information very seriously, so we sincerely apologize and regret any inconvenience this incident may cause. This notice contains information about what happened, steps we have taken, and the resources we are making available to those potentially affected.

What happened:

On February 8, 2021, Wolfe Eye Clinic was the target of a cybersecurity attack that involved an unauthorized third party attempting to gain access to our computer network. Upon detecting this incident, we moved quickly to secure our network environment and launched a thorough investigation. The investigation was performed with the help of independent IT security and forensic investigators to determine the scope and extent of the potential unauthorized access to our systems and any sensitive information. Given the complexity and scale of the cyberattack detected, the full scope of information potentially impacted was not fully realized until May 28, 2021.

What information was involved:

The comprehensive forensic investigation into this incident concluded on June 8, 2021. We discovered that the personal information of certain individuals may have been accessed by an unauthorized third party. For some, this information may include their name, mailing address, date of birth and Social Security number; and for others it may also include protected medical and health information.

What we are doing and what you can do:

We take the security of all information in our control very seriously. Given this, we are taking steps to prevent a similar event from occurring in the future by implementing additional safeguards and enhanced security measures to better protect the privacy and security of information in our systems. Out of an abundance of caution, we are also notifying all potentially affected individuals and have begun mailing letters to everyone whose information may have been compromised because of this incident.

To help guard against the possible misuse of information, we have secured the services of IDX to provide identity monitoring, at no cost, to affected individuals for twelve (12) months to help relieve concerns and restore confidence following this incident.
